Step 1 - Family Medical Release Form
Print and complete the Medical Release Form, one per family. Please turn this form in to the church by September 6.
Step 2 - Pre-Registration Form
Print and complete the Pre-Registration Form. This form will help us know what items (books, uniforms, etc.) your child/children will need when they arrive. Please turn this form in to the church along with payment by September 4. Paid pre-registrants will be able to receive their supplies on September 11.
Please make note of the suggested donation and actual costs for each child who will be attending Awana. These are also detailed on the pre-registration form (see Step 2).
Suggested Donation - $20
(covers partial costs of awards and other misc.)
Handbooks (Required Item) - $10
You will need a new Handbook if you are starting Awana for the first time, are entering a new club (i.e. Cubbies to Sparks or Sparks to T&T), are entering 5th grade in the T&T club, completed your handbook last year, or if you are a Cubbie (Cubbies need a new book each year).
Uniforms (Required Item) - $10 (Sparks), $5 (T&T)
You will need a new uniform if you are starting Awana for the first time or are entering a new club (i.e. Cubbies to Sparks or Sparks to T&T, 3-4th grade or T&T, 3-4th grade to T&T, 5-6th grade).
